I’m sorry to see that the bridge you have printed seems to not have fully adhered to the build platform and warped your piece, but I hope we can resolve this fairly quickly.
First off: You are also speaking of issues with your other models not printing well either, as this hints at a second issue with your printer, I highly advise opening a ticket with us in customer service (or giving us a call) so we can properly dive into your specific machine and help getting it fixed asap.
As some general advice for this:
(Partial) non adhesion can be caused by an improperly calibrated build platform height (Form 3 series) or a platform that is not perfectly parallel to the LPU (Form 4 series). You can find articles on fixing this here for the Z Axis fine tuning in case your printer is a Form 3 or here for aligning the build platform (Form 4 series).
Secondly, if you’re experiencing model dies that are too tight or too loose, I highly recommend the Precision Model Applications guide and in case you really want to make sure to tune for the perfect settings on your printer, you can do the fit tuning test. Instructions for that can be found following this link for Form 3 series and in case you’re using a Form 4, you can find the appropriate guide here.

Also had some recent fail, and the cause is simple: they want to promote Form 4, so they made Form 4 and its 0.05 mm step as the default for preForm, but I use Form 3L. Sad thing is that the step doesn’t automatically change from 0.05mm to 0.1 mm so the print failed due to the high failure rate of 0.05mm. Now I have spent one more day printing a trash and have to buy a new tank because the old one is broken by the failure.

I can absolutely confirm that we are not switching materials, settings or printers around in PreForm in order to confuse our customer base and/or guide people towards settings that perform worse than others.
If anything, we’d like everyone to always just use the most robust settings, the more people print, the better for Formlabs as a company (whichever printer is being used).
If your tank failed prematurely, please reach out to us in the services team and we will see what we can do about that.
About the issue of Preform going to different printers/settings etc.
The simple truth is that this is something that comes and goes from PreForm version to PreForm version and unfortunately breaks every couple of updates. Our preferred behaviour is for PreForm to simply reopen with the last used settings selected and the last printer you uploaded to also get preselected.
In PreForm 3.44.1 this functionality is unfortunately again broken and PreForm simply selects the printer type that we have set as default for now (Form 4 currently). We are investigating this issue and are hoping we can address this in the next PreForm release.
You can test this by switching to the dental workspace, where the “default” hasn’t been set to Form 4 yet and it’s still Form 3B so whatever printer you choose, close and reopen PreForm, it will open with the Form 3 Series selected.

Thanks Jakob, I have rolled back to 3.43.2 and will never update again. The tank is a little bit expired and we are awaiting for a new tank and printed in a hurry.

Okay, that’s valid. Make sure to go into the Help → Check for Updates settings menu and untick the “Automatically update PreForm to the latest version” otherwise PreForm will have updated to 3.44.2 again once you close and reopen it.
While I can not endorse purposefully keeping PreForm in an outdated state in general, I would recommend checking the PreForm update page from time to time to check if we fix this issue and then update specifically to that version.
A comparison would be PreForm 3.39.0 from August 2024, where you can find this exact problem in the “known issues” section.
